Removed firue 8 and performed laser therapy. Signifcantly reduced ROM, began PT. Reaced about 100 degrees at elbow ehn wound dehisced approxiatle 3cm in length. Appears skin is not healing to skin as skin margins were visible. Placed telfa pad with triple antibiotic and tegaderm. Figure 8 placed.

Concerned that skin margins are not healing to each other and may need to allow wound to heal via second intention

ROM of carpus normal, elbow significantly reduced. When attempting PT of the elbow, wound would start to dehisce. Did not push PT to due to risk of further dehisence. Figure 8 and bandage replaced. Patient recovered normally.

Surgery/PT:
019 was placed under general anesthesia.
Induced and maintained on isoflurane.
Heart rate and respiratory rate were monitored for the duration of the procedure.
Bandage was removed and the wound was cleaned and sterilely prepped for surgery.
The wound was closed in a 2 layer fashion. The first layer was closed with a simple continuous pattern with 5-0 PDS, and the second wound was closed with 4 simple interrupted sutures of 5-0 PDS.
Brief PT/PROM was performed post-operatively. Still reduced range of motion at the elbow joint. Adequate range of motion at the carpal joint.
A tegaderm pad was placed on the wound and the wing was wrapped in a figure 8 pattern with vetwrap.
Recovered from anesthesia smoothly.

After 019 was placed back in their kennel, 019 managed to remove the E-collar, and the wound on the right patagium was observed to have dehisced.
Another e-collar was secured around the neck.
Lube was placed in the wound to prevent contamination.
The wound was covered by a tegaderm patch and wrapped in vetwrap in a figure 8 pattern

019 was induced with 5% ISO at 1:45 pm via mask and was maintained on 3% ISO until 2:30pm in order to asses and resuture the wound on its right wing. Initial heart rate was 280 bpm, initial respiratory rate was 28 brpm. Heart rate was monitored via dopppler and respiratory rate was monitored through visualization through the whole procedure. The wound edges were debrided back to healthy tissue. The wound was thoroughly lavaged with warmed LRS prior to being sutured in a simple interrupted pattern with 5-0 PDS. Once the wound was sutured, a tagederm bandage was placed over the suture site and a figure 8 wrap was done on the right wing. E-collar still in place. Patient recovered from anesthesia uneventfully.

Patient tore bandage and tore some sutures. On evaluation of the suture site the middle area of the suture site had been opened, lubricant was applied to the exposed tissue and a tegaderm + pad was placed over top of the suture site. A figure 8 wrap was placed on the right wing and a size 7.5 hard e-collar was placed on the patient. Monitor q30 minutes to make sure we are not getting out of e-collar. Will put under anesthesia to evaluate suture site and re-appose tissue later this afternoon.

Initial heart rate was 300 bpm, heart rate was monitored via dopppler and respiratory rate was monitored through visualization through the whole procedure. A mixture of exudative material and necrotic tissue were cleared away from the wound that had been previously sutured on the right wing. The wound edges were debrided back to healthy tissue. The wound was thoroughly lavaged with warmed LRS prior to being sutured in a simple interrupted pattern with 5-0 PDS. Once the wound was sutured a tegaderm bandage was placed over the suture site. The right wing was placed in a figure 8 wrap. 019 recovered from anesthesia uneventfully and enrichment was placed in the cage to help deter them from ripping the bandage and sutures.

L wing - removed sutures, the majority of the sutured area is c/d/i, but the center had a suture missing and scabbing present that was inhibiting granulation, the area was cleaned and the protruding scab was removed, the area was then cleaned again and skin glue was applied to appose the skin edges

Head - healed well, removed sutures

R wing - there is an area where the skin edges have pulled apart, difficult to tell if sutures are missing but suspect they are, there is significant scabbing and crusting of exudate that will need to be debrided, removed a few sutures at the edges of the wound where it had healed sufficiently

Procedure - Wound debridement

Surgery time - 68 minutes

1:56 PM Initial HR: 296, RR: 24. Midazolam 2mg/kg IM was given
at 2:00PM. Patient was preoxygenated for 4-5 minutes before beginning
gas anesthetic. Gas anesthetic was started at 2:11PM. HR/RR were
monitored every 2-3 minutes. A heating pad was used to keep the
patient warm.  The wound on the right wing was debrided and any
devitalized (white, non-bleeding when cut) soft tissue was removed.
It was flushed with 20 mL of warmed LRS and was closed using
a series of 4-0 PDS sutures in a simple interrupted pattern.
The left wing wound was debrided, flushed, and closed in the
same manner. The extensor metacarpi radialis muscle was found
to be partially torn, and was sutured with 5-0 PDS.  The wound
on the back of the skull was debrided, flushed, and closed with
5-0 PDS in a simple interrupted pattern. During plucking of the
feathers around the wound on the back of the skull, a mild skin
tear occurred, which was also cleaned and closed with 5-0 PDS
with one simple interrupted suture. The wing wounds were both
covered with tegaderm to prevent the bird from removing the sutures.
Patient was extubated at 4:14PM and recovery was slow and uneventful.
Suture removal in 10-14 days.

Polytrauma from barbed wire entanglement

Assessment:
Today is day 4 of rehabilitation and treatment. On physical exam today, the laceration on the back of the head appears to be 7mm in diameter. It was cleaned with dilute betadine, applied sterile lube, and covered with tegaderm. The laceration on the left wing has moderate green bruising and soft tissue swelling distal to the elbow. Range of motion intact. The laceration on the right wing has bruising and swelling where feathers were pulled during the entanglement. No additional abnormalities were observed in either eye.

Plan:
1.Administer TMS (50mg/kg) 1.28 mL PO BID.
2.Administer Buprenorphine (0.1mg/kg) 0.08 mL SQ.
3.Meloxicam (1.5 mg/mL) 0.9 mL PO BID.
4.Monitor lacerations for signs of infection.

Assessment: 2024-019 presented tangled in barb wire. Following the removal of the barb wire, 3 lacerations were noted with one being on the right wing patagium, one on the left wing at the level of the ulna, and one on the back of the head at the base of the skull. Triple antibiotic ointment was placed on the lacerations and a tegaderm pad was placed over the left wing laceration. There was also a small retinal crate notated on the right eye but no other abnormalities with the eyes were seen. The pale mucous membranes could have possible been from blood loss while trapped.
Plan: 1. Submit HPAI test
2. Administer TMS (50mg/kg) 1.3 ml PO BID
3. Adminster Bupernorphine (0.1mg/kg) 0.07ml SQ
